The reason for this is simple:
The system only controls the line of English.
If I upload things and there is an English title, then I also insert it there. On E/EX it is usually the case that the title is in Romanji first and then in English.
The import system inserts the title in Romanji in the English line and then inserts the English title in Alternative names.
In this example, the system only searches for [Musume no Tomodachi no Mesugaki ni Okasaremashita 4]. But not for [I Was Raped by a Little Brat Who's Friends With My Daughter 4], under which I had uploaded it.
I got into the habit of just checking Pururin and the pending page for the Japanese name manually to see if it already exists here. But this only applies to things that are "slightly" older and could already be uploaded here.

On one hand it's going to miss stuff like this if either you or the other uploader bothered to do the names properly
And on the other hand it'll get very aggressive if you try to reupload anything that has been on pending but deleted because the image order got messed up or it got updated; Or if an existing gallery just happens to have the same name if it's something really generic like "Saimon Kanojo"/"Hypnotized Girl(friend)"
On one hand it's going to miss stuff like this if either you or the other uploader bothered to do the names properly
